For Students Entering Preschool
(Must be 2 yrs. by Sept. 1)

Complete the Registration Card (front & back).
-
Read & Sign Financial Contracts
-
Read Philosophy Statement, Appearance Code & Discipline Code.
-
Fill out Grandparent & Volunteer forms.
-
(New students) Attach birth certificate & verified shot record.
-
(New students) Must have a “Physician’s Report” signed by a physician
-
Return to secretary for approval.
-
Make payment to bookkeeper.
For Students Entering Kindergarten
(Must be 5 yrs. by Sept. 1)
-
Complete the Registration Card (front & back).
-
Read & sign Financial Contracts.
-
Read Philosophy Statement, Appearance Code & Discipline Code.
-
Fill out Volunteer & Grandparent forms.
-
(New Students) Attach birth certificate & verified shot record
-
(New Students using daycare) Have “Physician’s Report” signed by physician.
-
Return to secretary for approval.
-
Make payment to bookkeeper.
-
(New Students) a testing date will be set. A non-refundable testing fee of $65.00 is due at the time of registration or time of testing.

(New Students) A date will be arranged for the parent(s) to interview with the principal.
For Students Entering First through Fifth Grades
-
Complete the Registration Card (front & back).

Read & Sign Financial Contracts.

Read Philosophy Statement, Appearance Code & Discipline Code.

Fill out Volunteer & Grandparent forms.

(New Students using daycare) Must have a “Physician’s Report” signed by physician

(New Students) Copies of the following:
Immunization Record
(1st gr. only) birth certificate
Last 3 yrs. report cards
Last 3 yrs. achievement tests
Special testing for learning challenges
Return to secretary for approval.

For Students Entering 6th through Twelfth Grade

Read the Standard of Conduct & Statement of Cooperation and sign (both student & parent).

Read Philosophy Statement, Appearance Code, and Discipline Code.

(6th 11th New Students only) Student to complete the Student Questionnaire
-
(Students re-enrolling only) Student to complete the Spiritual Evaluation.
-
Parents read & sign financial contracts.
-
Complete the Volunteer & Grandparent forms. Make sure all information is complete.

(New Students) In addition to steps #1-6 you must provide copies of the following:

verified immunization record

last 3 yrs. report cards

last 3 yrs. achievement tests

Special testing for learning differences

(6th 12th only)Teacher Recommendations (English & Math teacher required)

(9th 12th only) Pastor Recommendation

Return to secretary for approval.
